Complete Your Consent/Authorization and Questionnaire and Schedule Your Worksite Biometric Screening Appointment
Have you received instructions from your employer to self-register for your MyHealth21 Dashboard account and you have never self-registered in the past? Follow the instructions below:
- Complete the online Health Assessment questionnaire.
- Schedule your worksite biometric screening and results consultation (if applicable to your program).
- You will be able to view available appointment times after you’ve completed the Health Assessment Questionnaire.
- Appointment reminders will be sent to the email address you provide.

- 1. Click Above to Login.
- Refer to the instructions you have received from your employer containing your login information.
2. Complete the online Health Assessment questionnaire.
3. Schedule your worksite biometric screening and results consultation (if applicable to your program).
You will be able to view available appointment times after you’ve completed the Health Assessment Questionnaire.
Appointment reminders will be sent to the email address you provide.
Integrated Health 21 Contracted Clinic Search
Locations & Contact Information
Participants unable to attend the worksite screening can search for the nearest IH21 contracted clinic prior to ordering an assessment kit.
Order Your In-Clinic Assessment Kit
Participants unable to attend the work site screening can order a kit and complete the health assessment/biometric screening at one of our contracted clinics nationwide. Participants must get authorization and an access code from their employer to use this option. Use the IH21 Contracted Clinic Location List to locate a clinic prior to ordering.
View Your Individual Health Profile
Participants can access their personal health report online (to view updates, print additional copies, etc.). This option is only available to participants after they have received their Individual Health Profile – information from the report is needed to create an online account.
Just for Employers
As an Integrated Health 21 client, you have access to a wealth of resources through this private employer portal. If you are a current Integrated Health 21 client, contact your Integrated Health 21 Account Executive to request your user IDs and/or passwords.
