Preventative Wellness Evaluation: New & Upgraded Services for 2025

The Preventative Wellness Evaluation offers each employee an opportunity to undergo a comprehensive and detailed health risk assessment conducted by a Certified Registered Nurse Practitioner (CRNP).

Our CRNP will compile a comprehensive health profile, tailored specifically to each individual, and arrange personalized consultations to discuss results.

Through our Preventative Wellness Evaluation, employees gain invaluable insights that empower them to make informed decisions about their health.

Provider Evaluation
IH21’s clinical team conducts a thorough assessment, encompassing various elements such as two (2) Mental Health screenings (Depression and Anxiety), measurements of Height, Weight and Body Mass Index (BMI). Blood Pressure evaluation, an Expanded Blood Work Panel, and comprehensive questionnaires covering past medical history, family medical history, current diagnoses, current medications, allergies, surgical history and anthropometrics.

Provider Referral
During a virtual appointment, the CRNP will interview the employee and discuss opportunities to improve health. Together, a comprehensive health and wellness plan is created, and necessary referrals, recommendations, and resources are given to the employee.

Nurse Practitioner Follow-Up
Often times this evaluation is followed by a 4 and/or 12-week follow-up to offer assistance as the employee strives to follow the Wellness Plan. This follow-up provides a forum for employees to give updates and feedback about their experience and ask for help if needed.
